Re:     Funding Application Materials for FY 2018 

In the links below, you will find the materials needed to complete the FY 2018 Application for Funding from the Madison County Mental Health Board. The Mental Health Board follows a three year funding cycle with a larger set of forms in year one and decreased materials in years two and three. FY 2018 marks the beginning of a new three-year cycle and therefore the larger package of materials is being used for all applicants.

Please complete all application materials as thoroughly as possible (a checklist has been included for your convenience). Be as descriptive as possible in the narratives, identifying a profile of individuals benefiting from 708 funding and clearly name service outcomes. If available, include any local statistics/data pertinent to the needs of the populations you are serving.

Materials in each of the links cannot be saved online. Save the files to your computer. Due to formatting issues of past years and software limitations, all materials are in Microsoft Word files. We want to make the application process as smooth as possible for everyone. If you find it necessary to make some additions/minor adjustments to the documents, such as adding lines to the financial forms, please do so. We ask that you do not completely reformat the documents or make any extreme content changes. You may also attach any supplemental materials you wish, that represent your agency and programming.

The Board cannot assure that all requests will be funded. The Board will be reviewing its funding priorities to ensure that core services can be continued. Applications are due by 12:00 p.m. on Friday, July 21, 2017.
