In the links below you will find the materials needed to complete the FY 2016 Application for Funding from the Madison County Mental Health Board. Because the Mental Health Board follows a three year funding cycle, there are two different funding application packages. For Current Service Contractors, this is a renewal/shorter application year – please use the “Current Contractor” link for the application materials. For New Applicants, you will need to use the new/longer application link for your materials. If you are not sure which category your agency would be; please refer to the Mental Health Board website page “Contracted Service Providers” for the list of current contractors (this does not include agencies that have received only System Development funding as listed on the “Contracted Educational/Training Providers” page).
Please complete all application materials as thoroughly as possible. Materials in each of the links cannot be saved online. Save the files to your computer. Due to formatting issues of past years, all materials are in Microsoft Word files. We want to make the application process as smooth as possible for everyone. If you find it necessary to make some additions/minor adjustments to the documents, such as adding lines to the financial forms, please do so. We ask that you do not completely reformat the documents or make any extreme content changes. You may also attach any supplemental materials you wish, that represent your agency and programming.
The Board cannot assure that all requests will be funded. The Board will be reviewing its funding priorities to ensure that core services can be continued.
Applications are due by 12:00 p.m. on Friday, July 31, 2015.
