2016-17 BENEFITS OPEN ENROLLMENT

Open enrollment is an annual event where employees are allowed to make changes to their benefit plan elections. Eligible employees may enroll into a plan, terminate from a plan, change from one plan to another and add/drop dependents during this window. The benefits that are chosen by the employee during this open enrollment window must be continued through the entire plan year unless a qualifying event takes place that permits a mid-year election change.

OPEN ENROLLMENT WINDOW: OCTOBER 4, 2016 through OCTOBER 18, 2016

EFFECTIVE
 DATE OF OPEN ENROLLMENT CHANGES:
1. Medical, Dental, Vision, HSA, UNUM Supplemental Accident and/or Critical Illness: 
    December 1, 2016 (Plan Year 12/1/16 - 11/30/17)
2. Flexible Spending Account (FSA): JANUARY 1, 2017 (Plan Year 1/1/17 - 12/31/17)

GENERAL INFORMATION:
* There are medical plan design and rate changes. Review your OE Packet, the Online System, or
   click on the links below for more information.

* Delta Dental Rates are reducing 4.5% and a coverage benefit is added for mouth guards (for teeth
   grinding) in Major Restorative Service Coverage at 50%.
* Vision Rates are reducing 5%. No coverage changes.
* FSA elections must be made each year. They do not carryover from the previous year.
* One deadline for both Medical and FSA open enrollment this year; Deadline: 10/18/2016.
* Employee Assistance Days are scheduled.

* Employee Meetings are scheduled. See Employee OE Meetings 

WHAT YOU NEED TO DO:
1. Review the information in your Open Enrollment Packet, or by clicking on the links below. 
2. Mark your calendar and attend one of the Employee OE Meetings
3. Decide which plans you want and which dependents you want to cover. 
4. Decide if you would like to save tax dollars by enrolling in an FSA or HSA plan.
5. Log into the Employee Self Service Make your medical, dental, vision, FSA, and HSA enrollment          elections between October 4, 2016 and October 18, 2016.
6. Enrollment Advisors for the Supplemental Accident and Critical Illness insurance will be in
    the office on October 12th, 13th, and 14th. If you were hired on or after 10/1/2015, you are
    newly eligible and must contact Penny Dummitt (pldummitt@co.madison.il.us or 296-5516 to
    schedule an appointment. Employees who were previously eligible may also schedule an 
    appointment.
    Deadline to schedule your appointment is 10/7/16
7. If you need personal assistance, attend one of the Employee Assistance days.
