What to bring to your vaccination appointment:
- Photo ID with birthdate for all ages. If using a school ID, bring birth certificate.
- Parent or guardian (with proof of guardianship) if 16 or 17 years old.
- Proof of employment or residency in Illinois.
- Vaccination card for 2nd dose.
All vaccinations are By Appointment Only and must meet the Criteria below.
- Must be 16 years of age or older
- Must show proof of Illinois residency or Illinois employment
If you are 16 or 17, you must be accompanied by a parent and bring a photo ID. Pfizer is the only manufacturer approved for this age group.
Even if you completed our January survey and meet this criteria, please feel free to schedule an appointment.
2nd Dose appointments are for people who received their 1st dose through a Madison County Health Department Vaccination Clinic and are due for their 2nd dose of the same vaccine brand.
Bring your Vaccination Card and your photo ID to your 2nd dose appointment.
If you do not have computer access, call 618-650-8445 for help scheduling an appointment.
